All owners are requested to submit an Owner Contact Information Form.

The Alteration, Repair or Construction Form (ARC) and Guest Registration form can be completed either online version (no printing, scanning, etc. necessary) OR, by choosing the fillable PDF form which can be printed and submitted by scanning and emailing to luci.guillen@fsresidential.com or sending via US Mail to Luci Guillen, FirstService Residential, 10600 Chevrolet Way, Suite 200, Estero, FL 33928.
Renters or Purchaser Forms
Attention Renters or Purchasers: All potential renters or purchasers of a Harborside Terrace Owner's Association (HTOA) unit must complete the Tenant Evaluation online background check. The form requires property code " 8700". Please be advised that it may take 10 days to 2 weeks for this form to be processed.
Single Person Fee: Any unmarried/single person must submit a separate background application at the cost of $150, regardless if there are multiple persons on the lease.
Married Couple Fee: The application allows a married couple to complete ONE application for the cost of $150. Married couples whose last names are not the same are required to upload a copy of their marriage certificate in order to qualify for the $150 fee. If applicants are married, have different last names and do not upload the marriage certificate will be charged DOUBLE ($300).
